If you live with diabetes, you’re probably used to tracking a lot of numbers.
HbA1c. Time in range. Lipids. Blood pressure. Weight.
But there’s one organ that often gets left out of the conversation. The brain.

What is Tau-217 (and why should you care)?
Tau-217 is a specific phosphorylated form of the tau protein. In Alzheimer’s disease, tau proteins misfold, clump together, and disrupt communication between neurons. For years, the only reliable way to detect this process was through PET scans or spinal taps. Expensive, invasive, and not exactly screening-friendly.
Tau-217 changes that.
Recent studies show that blood-based tau-217 testing can detect Alzheimer’s pathology years before symptoms appear, with accuracy approaching that of PET imaging. In other words, we can now see the biological footprint of Alzheimer’s early and quietly, through a blood test.
That’s a big deal.
Why this belongs in diabetes care
People with diabetes face a higher risk of cognitive decline due to chronic inflammation, vascular damage, insulin resistance in the brain, and repeated glucose fluctuations. These processes don’t just affect blood vessels, they affect neurons.
So when we think about diabetes care, the question becomes:
Are we only managing blood sugar… or are we protecting long-term brain health too?
Testing tau-217 isn’t about diagnosing Alzheimer’s on the spot. It’s about risk stratification. It helps answer a more nuanced question:
Is neurodegeneration already quietly underway?
For people living with diabetes, that information matters because the earlier you identify risk, the more leverage you have.
Early detection changes the game
There’s no sugarcoating this: we still don’t have a cure for Alzheimer’s.
But what we do have is a growing list of interventions that can meaningfully slow progression when applied early.
Metabolic control.
Sleep optimization.
Cardiovascular risk reduction.
Exercise (especially resistance training).
Weight loss when appropriate.
Addressing insulin resistance aggressively.
Tau-217 gives us a biological signal that helps guide how urgently and aggressively we act, long before memory loss shows up in daily life.
So why are we integrating this now?
Because diabetes care can’t be siloed anymore.
If we know that metabolic disease and neurodegeneration are deeply connected, then it makes sense to assess them together. Tau-217 allows us to bring brain health into routine diabetes care in a way that’s practical, evidence-based, and proactive.
Not everyone needs this test.
But for the right person, at the right time, it can be incredibly informative.
